Initially misinterpreted as a hernia, the patient underwent venogram and intravascular ultrasound revealing severe venous stenosis. Despite stent placement, recurrence of bursitis led to repeat procedures. Ultimately, a CT scan identified a fluid collection causing stent compression. Aspiration of the fluid resolved the compression, leading to symptom improvement. The report emphasizes the importance of recognizing extrinsic venous compression and obtaining additional imaging for accurate assessment. Conservative treatments failed, necessitating stent placement to relieve chronic venous obstruction. While the patient showed improvement and underwent hip revision surgery, earlier identification of iliopsoas bursitis could have altered management. The study highlights the significance of employing various imaging modalities for comprehensive diagnosis and appropriate treatment of similar cases.
