Accuracy of Duplex Ultrasound for Detecting Renal Artery Stenosis: A Systematic Review

A systematic review examined the accuracy of Doppler ultrasound (DUS) in detecting renal artery stenosis (RAS) compared to angiography. The review included 34 studies from after 2005, with varying definitions of stenosis and ultrasound parameters. DUS showed moderate to high agreement with angiography, but lower agreement in studies with smaller sample sizes or unique stenosis definitions. The review suggests standardizing study reporting, stenosis definitions, and statistical methods for future research. While DUS demonstrates promise as a noninvasive and potentially cost-effective method for RAS detection, further research is needed to determine optimal diagnostic parameters and thresholds. Standardizing methodologies, assessing cost-effectiveness, and considering practical implementation factors are recommended. The review supports the use of DUS as a first-line screening tool for RAS, with angiography for inconclusive cases, and highlights the need for more research and consensus in this area.
