Video Summary
The SVU e-Learning webinar presented a series of diverse vascular ultrasound cases by experts from across the United States, emphasizing learning from real-life clinical challenges.

The first case involved an unusual carotid artery transposition surgery after recurrent stenosis post-endarterectomy, highlighting the decision-making toolbox for treating carotid disease beyond standard stenting or endarterectomy. The complex anatomy required innovative surgical techniques with favorable outcomes.

Next, a case of Bowhunter's syndrome—rotational vertebral artery occlusion—showed the importance of reproducing patient symptoms during vascular testing. By mimicking occupational neck positions, the lab identified vertebral artery flow changes and confirmed vertebrobasilar insufficiency, reinforcing tailoring exams to patient history.

A young female with median arcuate ligament syndrome (MALS) had chronic postprandial pain and high celiac/SMA artery velocities that normalized with deep inspiration. Imaging and serial velocities pre- and post-ligament resection demonstrated the vascular compression and partial improvement after surgery.

A rare horseshoe kidney case underscored the need to consider abnormal renal anatomy when scans fail to visualize kidneys conventionally. Thorough sonographic exploration identified the fused renal masses and isthmus bridging.

Another patient post-transcoronary aortic dissection had an IVC filter placed for venous thromboembolism protection. Follow-up ultrasound revealed thrombus on the filter, emphasizing vigilance in duplex imaging of filter patients to assess thrombus and filter position.

A superior mesenteric artery dissection with thrombosis was managed conservatively with fluid, anticoagulation, and monitoring. Imaging documented flow compromise and thrombus presence, illustrating rare mesenteric vascular pathology.

Lastly, a young man with venous insufficiency symptoms was found to have iliac vein compression (May-Thurner syndrome) causing severe leg symptoms. Ultrasound and intravascular ultrasound demonstrated complete iliac vein obstruction relieved with angioplasty and stenting. This case highlighted the importance of critical clinical and sonographic assessment beyond protocols.

Overall, the webinar showcased the value of case-based learning, comprehensive imaging protocols, and clinical correlation in vascular ultrasound practice. Panelists encouraged continued sharing of challenging cases to advance education and patient care.
