Dr. Akram Esbuta discussed his research on extracranial internal carotid artery (ICA) Doppler ultrasound imaging findings in Kuwaiti children with sickle cell disease (SCD) but without neurological symptoms. His study, involving 59 steady-state SCD patients, found no abnormal time average mean velocities (TAMV) indicative of increased stroke risk in intracranial or extracranial ICAs, likely due to elevated fetal hemoglobin (HBF) levels in this population. There was a strong TAMV correlation between the two ICAs but poor correlation between intracranial and extracranial ICAs. TAMV in the extracranial ICA correlated with age and some hematologic parameters linked to disease severity, suggesting further research is needed on stroke development mechanisms in these vessels. The talk highlighted advancements from non-imaging to imaging transcranial Doppler and the importance of extracranial artery evaluation.<br /><br />The second presentation by Diane Massery was a case study of delayed arterial injury after a gunshot wound to the right forearm in a 20-year-old male. Despite initial surgical repair, he presented eight weeks later with pain and swelling. Imaging including duplex ultrasound revealed a large ulnar artery pseudoaneurysm with abnormal flow patterns. Ultrasound-guided thrombin injection failed, necessitating referral for surgical repair. The case emphasized the importance of surveillance after penetrating injuries since delayed vascular complications can occur even if initial evaluations are negative. Ultrasound proved critical for early detection of pseudoaneurysms and planning management. Both presentations underscored the vital role of imaging in vascular pathology diagnosis and follow-up.
